Information clause
In accordance with Article 13(1)-(2) of Regulation (EU) 2016/679 of the
European Parliament and of
the Council of 27 April 2016 on the protection of natural persons with regard to the processing of
personal data and on the free movement of such data, and repealing Directive 95/46/EC (General
Data Protection Regulation) (Official Journal of the EU L 119, p. 1) (”GDPR”), we hereby inform you
that:
1. The administrator of your personal data is “WOM&AN” (National Court Register no. KRS
0001050813) with its registered office in Warsaw at ul. Sienna 39, 00-121 Warsaw, Poland
(“Administrator”).
2. The Administrator can be contacted at the e-mail address [email protected]
and/or by regular mail sent to the Administrator’s registered office address indicated in Section 1
above.
3. The Administrator processes the following personal data:
a) name and surname,
b) e-mail address,
c) image – if consent is given.
4. Your personal data may be processed:
a) in order to verify that a candidate for membership in the Association meets the requirements for
membership, as defined in the Association’s Articles of Association, and in connection with the
membership relationship in the Association (Art. 6(1)(b) GDPR – processing is necessary for the
performance of a contract to which the data subject is a party or to take action at the request of
the data subject before the conclusion of the contract),
b) to assert claims, if any, arising in connection with the membership relationship (Article 6(1)
(f) GDPR – legitimate interest of the Administrator),
c) to fulfill a legal obligation incumbent on the Administrator (Article 6(1)(c) GDPR),
d) as regards your image and marketing communications – to promote “WOM&AN” and its
activities (Article 6(1)(a) GDPR – consent of the data subject).
5. Your submission of personal data is voluntary but necessary to join the Association as a
member.
6. Personal data will be processed for the period necessary, but not longer than necessary for
processing. In any case, your data will be processed not longer than until an effective
objection is made. In connection with defense against claims, data may be processed until
expiration of the applicable statute of limitations.
7. You are entitled to:
(a) The right to access your data and receive a copy of it,
b) The right to rectify (amend) your data,
(c) The right to erasure and to restriction of data processing,
(d) The right to object to the processing of your data,
(e) The right to data portability.
8. In accordance with Article 17 GDPR, you have the right to demand that the Administrator
deletes your personal data forthwith in the event of revocation of membership.
9. The following may be recipients of your personal data:
a) entities providing services to the Administrator such as legal, administrative, IT, archiving,
and accounting services;
b) authorized employees, associates, and members of the Administrator.
10. With regard to your consent for image processing, you have the right to withdraw your consent
at any time. Withdrawal of consent does not affect the lawfulness of processing carried out
based on your consent before its withdrawal.
11. If you believe that the Administrator’s processing of your personal data is in violation of
GDPR,
you have the right to lodge a complaint with the Polish Office for Personal Data Protection.
12. We would also like to inform you that the Administrator makes every effort to apply all
physical, technical, and organizational measures to protect your personal data from accidental
or intentional destruction, accidental loss, alteration, unauthorized disclosure, use, or access
in accordance with all applicable laws.
